Community Health Paramedic Program

The Robertson County EMS Community Paramedic Program is a specialized initiative that works directly with patients, families, and healthcare providers to bridge the gap between emergency response and ongoing healthcare needs. Through in-home visits and personalized support, the program helps patients better understand and manage chronic physical and mental health conditions, reduce avoidable use of emergency services, and connect with community resources. By focusing on education, advocacy, and prevention, the program promotes improved health outcomes, greater independence, and a higher quality of life for vulnerable members of our community.

The RCEMS Community Paramedic Program is designed to improve patient health and reduce unnecessary emergency service use through proactive care, education, and support. The program works closely with identified patients and their healthcare providers to:

  • Provide education on chronic disease management and nutrition
  • Support medication management and safe usage
  • Treat certain exacerbations of chronic conditions under medical direction
  • Coordinate care with other healthcare providers, agencies, and community resources
  • Offer fall prevention education and in-home safety assessments
  • Assist with navigation to appropriate services and programs
  • Advocate for patients and strengthen communication between patients and providers

Through these efforts, the program promotes healthier outcomes, reduces hospital visits, and helps patients live safer, more independent lives.
